Responsibilities:

  • Assist in planning, developing, organizing, implementing, evaluation, and directing the social service programs of the facility.
  • Keep abreast of current federal and state regulations, as well as professional standards, and make recommendations on changes in policies and procedures to the Administrator.
  • Participate in community planning related to the interests of the facility and the services and needs of the resident and family.

  • Assist in developing a written plan of care (preliminary and comprehensive) for each resident that identifies the problems/needs of the resident and the goals to be accomplished for each problem/need identified.
  • Ensure that social services components of the MDS are completed and signed in a timely manner.
  • Review nurses’ notes to determine if the care plan is being followed. Report problem areas to the DON.
  • Assist in providing solutions for social and practical environmental problems including seeking financial assistance, discharge planning (including collaboration with community agencies), and referrals to other community agencies when specialized assistance is required.
  • Assist Director of Social Services with admissions process and paperwork as needed.
  • Assist in standardizing the methods in which social services tasks will be performed. 
  • Supervisor may assign additional duties as necessary.

Qualifications:

  • Possess, as a minimum, a Bachelor’s Degree from an approved school of Social Work, or some other human services related field.
  • Ability to make independent decisions when circumstances warrant such action, and to deal tactfully with personnel, residents, family members, visitors, and the general public.
  • Must be knowledgeable of administrative practices, procedures and guidelines
  • Excels at managing multiple priorities

  • Maintain an adequate inventory of office and therapy supplies, order through DSSI and facility.
  • Assist with scheduling of meetings and appointments within therapy department.
  • Assist with Medicaid submission and tracking authorizations as needed.
  • Agree not to disclose resident’s protected health information and promptly report suspected or known violations of such disclosure to the Administrator.
  • Maintain an excellent working relationship with the medical profession and other health related facilities and organizations.
  • Develop and maintain a good working rapport with inter-department personnel, as well as other departments within the facility to assure that services and activities can be properly maintained to meet the needs of the residents.
  • Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals in accordance with the facility’s policies and procedures governing accidents and incidents.
  • Additional duties may be assigned by your supervisor(s).

Qualifications:

  • Certification from a Nursing Assistant Program and listed on the C.N.A registry with current State and Federal eligibility.
  • Knowledge of general computer skills, Word, and Excel.

  • Assist the Quality Assessment and Assurance Coordinator in developing, implementing and maintaining an ongoing quality assurance program for Occupational Therapy services.
  • Maintain an adequate liaison with families and residents.
  • Evaluate and implement recommendations from established committees as they may pertain to Occupational Therapy services.
  • Maintain an excellent working relationship with eth medical profession and other health related facilities and organizations.
  • Create and maintain an atmosphere of warmth, personal interest and positive emphasis, as well as a calm environment throughout the department.
  • Assist with an effective orientation program that orients the new employee to the facility, its policies and procedures and to his/her job position and duties.
  • Be alert for resident safety during any/all therapy.
  • Ensure that the therapy area is maintained in a clean and safe manner for resident comfort and convenience by assuring that necessary equipment and supplies are maintained to perform such duties/services.
  • Recommend to the D.O.R the equipment and supply needs of the department.
  • Assist in the Occupational Therapy budget and submit to the D.O.R as directed.

Qualifications:

  • Must possess, as a minimum, a Bachelor’s Degree in Occupational Therapy from an accredited college or university.
